Hamilton Memorial Home Health Agency offers a range of services provided by Registered Nurses, Home Health Aides, Physical Therapists, Speech Therapists, Occupational Therapists, and a Social Worker. All services are performed under the direction of your physician.

Care provided by the Registered Nurse is directed towards:

  • wound care
  • catheter maintenance
  • diet therapy
  • ileostomy, colostomy care
  • patient education
  • lab work (blood, urine, etc)
  • medication monitoring
  • teaching basic nursing care
  • teaching disease process
  • diabetic care
  • IV Therapy

Care provided by the Aide is directed toward:

  • assisting in personal care
  • bathing
  • skin care

Care provided by the Physical Therapist is directed toward:

  • theraputic exercises
  • muscle strengthening
  • transfer training
  • gait training
  • muscle re-education skills

Care provided by the Speech Therapist is directed toward:

  • retraining of language
  • retraining in swallowing
  • retraining in hearing
  • retraining in memory

Care provided by Occupational Therapy

  • evaluate physical setting
  • evaluate physical/mental limitations and assets
  • recognition and prevention of disabilities and restoration toward normal health and function
  • coordination of activities with community agencies
  • continued therapy after hospitalization
  • adjustments and modifications in daily activities

Care provided by the Medical Social Worker

  • assess and help client's social and emotional needs
  • assist client advance directives
  • assess financial needs and available resources
